*Description*
The SWICA invoice app provides insured persons with a fast, convenient and secure way of using their smartphone or tablet to photograph medical bills and other related documents and send them directly to SWICA. This means that SWICA customers always know which bills they have submitted and can easily track the status of their claims.

*Conditions*
• You must be a customer of SWICA Healthcare Insurance Ltd and have an active insurance number
(i.e. an active insurance contract) or the contract must have expired no more than six months ago.
• You must have active basic or supplementary insurance cover with SWICA Healthcare Insurance Ltd.
• You must be legally domiciled in Switzerland or you are recognised as a cross-border commuter under the bilateral accords.
• You must have an active email address
• iPhone 5, iOS 8 and later
• iPad Air 2, iOS 8 and later

*Registration*
The User must register for the service (i.e. activate it) in the SWICA invoice app. The following information is required: user name (i.e. email address), password, insurance number, date of birth, and mobile phone number. The User is then sent an activation link.
The User gives an undertaking that all the data provided during registration is complete and accurate.

*Retention of original documents*
The User is obliged to retain all original documents and to provide them to SWICA on request. SWICA expressly reserves the right to refuse to disburse benefits or to claw back benefits which have already been disbursed if the relevant original documents cannot be produced on request.

*Security notes*
The User is aware that the SWICA invoice app is available from app stores and that this is an open, freely accessible network. Despite state-of-the-art security technologies it is not possible to ensure absolute security on SWICAs side, on the Users side, or when data is being transmitted.
SWICA urges the User to take all necessary security measures on his/her computer (e.g. by installing up-to-date anti-virus software and a firewall) and, in particular, not to store health-related data locally on devices (e.g. computers and mobile devices) to which third parties could gain access. The User must also bear the risks which arise as the result of unauthorised manipulation of his/her computer, or as the result of misuse of his/her login information and/or access data, or during data transmission.

*Legal information*
SWICA will use its best endeavours to ensure the correct operation of the SWICA invoice app and the electronic services. SWICA offers no guarantee, however, that the SWICA invoice app will be available at all times or that it will be error-free.To the extent possible under the law, SWICA rejects all liability for direct or indirect losses which the User may suffer in connection with using the SWICA invoice app.

Moreover, SWICA accepts no liability of any kind for damage to the Users devices (e.g. mobile phones, tablets) or the data stored on them resulting from inadequate technical precautions, unauthorised manipulation, overloads, malfunctions, other problems or unlawful interference with telecommunication facilities or computer systems.

Downloading and using this application can result in mobile phone charges.
